OUR STORY.

In 2020, while running a physician group that worked alongside skilled nursing facilities and home health agencies, our founders identified a disconnect between the quality of care patients received during an in-patient stay and that available post discharge.
This gap created an opportunity for disruption; a chance for industry leaders to come together and dream up a new model of home healthcare. One that champions outcomes using patient-specific data and leverages integrated technology – instead of legacy protocol – to inform patient care.
And so Home Care Experts (HCX) was born. Gathering together a group of healthcare executives from across the industry spectrum, HCX was conceived out of the desire to serve patients the way they should be cared for after leaving a skilled nursing facility or acute hospitalization so they can recover safely at home. HCX partners with physicians, skilled nursing facilities, hospitals, health systems, and insurance companies who share our desire to champion individualized care plan management and have the capabilities to employ patient data and the latest technology in care. Together, we help patients recover faster and ultimately, achieve the best possible outcomes while reducing overall costs.
